ILLNESS CERTIFICATE REQUEST
You may be eligible for Illness Benefit from the Department of Employment Affairs and Social Protection (DEASP) if you're unable to work due to illness. To apply, you must obtain a medical certificate, known as a ‘Certificate of Incapacity for Work,’ from your GP. Your GP will complete the certificate and can submit it electronically through a secure system. In some cases, one certificate may cover the entire period of your illness; in others, you might need to return for further evaluation.

Additionally, your employer may ask for a separate medical certificate (often referred to as a ‘sick note’ or ‘work cert’) if you are on sick leave. For instance, a certificate may be required if you are off work for more than 2 or 3 consecutive days. Please remember that a GP must issue your illness benefit certificate or medical certificate. If you need to extend your certificate, you can submit the form below, and we will reach out if a further review by your GP is necessary.
The online illness certificate request fee is €20 and must be paid prior to processing your request.
